Wordpress Plugins / Error after a feedback post referring to EasyChart...

Any chance you'd take a look at why this error is being thrown??

After submitting this page:
http://inwallreviews.com/ask-us-to-review-your-speakers/

It sends the form ok, but then arrives with this message:
